Any of our events that require paper consent/booking forms will be available here near to each event date; eg. pupils' disco.

All cheques are payable to:  ‘FRIENDS OF ST PETER’S JUNIOR SCHOOL RUDDINGTON’

Please remember to complete all sections of the booking form for FOS events as this helps safeguard against any mistakes or mix ups. You need to complete the following:-

-no. of tickets

-total cash amount

-how your child(ren) are being collected at the end

-any diet or other needs requests

-childs name

-class

-parents name
